About the Role 

Across Home Health, we’re transforming the way we work in local communities to improve health and care outcomes. As the Health Concierge Team Leader - Clinical, you’ll play a pivotal role in shaping how we deliver personalised and integrated care. Leading a virtual team, you'll ensure efficient triaging, scheduling, and workforce planning to enhance the delivery of high-quality allied health and nursing services. 

This is a permanent full-time, hybrid role based in Western Australia, with flexibility to work from home while maintaining the ability to attend our Delhi Street, West Perth office as required.

The Day to Day

In this dynamic role, you will: 

  • Oversee customer intake, triaging, and clinical referral assessment 
  • Manage workforce scheduling and operational rostering to maximize efficiency 
  • Ensure the right resource meets the right need at the right time 
  • Provide leadership in referral management, insourcing/outsourcing decisions, and team performance 
  • Leverage data-driven insights to optimise care delivery and workforce utilization 

 
About You

We’re looking for a clinical and operational expert with strong business acumen and a passion for improving care outcomes. You’ll thrive in this role if you: 

  • Hold tertiary qualifications in allied health or nursing with current AHPRA registration 
  • Have significant experience in clinical operations and workforce management 
  • Are data-focused, process-driven, and always seeking efficiency gains 
  • Possess strong leadership skills to inspire and guide a high-performing team 
  • Understand how to balance customer needs with optimizing service delivery
