Physical Therapist (PT) | Part-Time
All Heart Home Care | Columbia, TN
All Heart Home Care is seeking compassionate and skilled Physical Therapists Part-Time to join our growing home health team in the Columbia, Tennessee area. This role provides the opportunity to deliver personalized, one-on-one care to patients in the comfort of their homes while enjoying a flexible schedule and a supportive work environment.

Key Responsibilities
- Perform in-home evaluations to assess patient needs and develop individualized treatment plans
- Provide skilled physical therapy services to improve mobility, strength, balance, and overall function
- Monitor patient progress and modify treatment plans as appropriate
- Educate patients and caregivers on home exercise programs, safety measures, and injury prevention
- Complete accurate and timely clinical documentation in accordance with company policies and state regulations
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care
- Travel to patient homes within the assigned service area
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
- Active Tennessee Physical Therapist license
- Current CPR certification
- Valid driver's license, reliable transportation, and current automobile insurance
- Strong clinical assessment and communication skills
- Ability to work independently and manage time effectively
- Previous home health experience preferred, but not required
