Summary:
We're looking for a versatile team member to fill a new hybrid role that blends front-office support with hands-on patient care. In this position, you'll primarily staff the front desk as our home health office receptionist, while also serving as a certified CNA available to complete field visits as patient care needs arise (field visits are typically completed within 45 minutes to a maximum of 1 hour.)
Responsibilities:
Receptionist/Administrative Duties:
- Conducts self in a professional and courteous manner
- Serves as receptionist and permits only authorized personnel beyond the reception area
- Answers the telephone and receives messages in a pleasant, professional manner
- Receives and submits faxed information
- Ensures confidentiality of all Protected Health Information
- Ensures faxed information is submitted to the appropriate person
- Assists in the production and completion of requested documents, including plans of care and other documents as requested
- Ensures all documents produced are grammatically correct and properly spelled
- Ensures all office equipment is working and properly maintained
- Checks eligibility for patients
- Enters patient data
- Schedules visits
- Prepares invoices for payroll
- Gathers needed information for private billing and faxes it to billing
- Coordinates with private insurance companies for authorization
- Checks HMO status on the first of each month for all patients, prints and files 3 pages per patient
- Adds new physician information into the system after collecting NPI and other required information
- Performs miscellaneous office tasks such as faxing, copying, answering phones, maintaining Excel spreadsheets, drafting memos, and other duties as assigned
CNA/Field Visit Duties:
- Provides safe and effective care to the sick and elderly
- Takes accurate temperature, pulse, respiration, and blood pressure
- Performs simple procedures as an extension of the therapy service, as directed
- Follows Universal Precautions and washes hands before and after each patient visit
- Completes all work assignments and turns in notes as required by Agency policy
- Understands and adheres to established Agency policies and procedures
- Adheres to Agency standards and consistently interprets and accurately performs assigned responsibilities
- Maintains acceptable attendance
- Reports incomplete work assignments to the RN
- Maintains a clean, well-groomed appearance
- Demonstrates effective time management through daily documentation
- Attends position-related in-services, case conferences, and staff meetings
- Attends all mandatory in-service programs as scheduled
- Completes a minimum of 12 hours of in-service training per calendar year (based on date of employment)
- Maintains a clean and neat work environment
- Demonstrates sound judgment and decision-making
- Coordinates care with the RN and reports changes in patient condition
- Participates in the Agency's QAPI meetings
