About the Role

Preventive care only works if it actually happens — screenings get done, records get gathered, and nothing falls through the cracks before the patient ever sits down with their doctor. As our Clinical Medical Assistant – Value-Based Care, you'll be the person making that happen, working on-site within 1–2 independent primary care practices in Queens to strengthen pre-visit planning, preventive screening, and Annual Wellness Visit support.

This is a role built for someone who wants to grow. Whether you're an experienced MA or new to healthcare, if you're organized, curious, and eager to learn, you'll have real ownership and a clear path to developing new clinical skills from day one.

Our practices serve a significant Korean-speaking patient population, so Korean language proficiency is required for effective, comprehensive patient communication.

What You'll Do

Pre-Visit Planning & Patient Preparation

  • Review upcoming schedules and records ahead of appointments
  • Conduct pre-visit chart review to flag preventive care gaps and missing records
  • Gather outside records, screening results, specialist reports, and labs
  • Support medication reconciliation and other patient prep
  • Ensure completed services and clinical information are properly captured in the EHR

Annual Wellness Visits & Preventive Care

  • Assist with preparation and completion of Medicare Annual Wellness Visits (AWVs)
  • Support Health Risk Assessments and other preventive visit components
  • Help identify patients due for preventive services
  • Support outreach, scheduling, follow-up, and documentation for preventive care
  • Assist with breast cancer, colorectal cancer, diabetes-related, and immunization initiatives

Patient Screening & Clinical Testing (after training, under practice protocol)

  • Administer or assist with PHQ-9 depression screening
  • Support cognitive screening (e.g., MMSE) using approved tools
  • Assist with spirometry and pulmonary function testing
  • Support diabetic retinal/eye screening using office equipment
  • Take vital signs and other routine measurements

Value-Based Care & Care Gap Support

  • Identify preventive and quality care gaps through pre-visit review
  • Flag opportunities to providers and practice teams
  • Track completion of recommended screenings, testing, referrals, and follow-up
  • Support documentation of completed screenings and preventive services

Practice Workflow Improvement

  • Work with physicians and staff to build efficient pre-visit and preventive-care workflows
  • Help practices adopt new screening tools, equipment, and processes
  • Identify practical workflow improvements and communicate them to leadership

Traditional Medical Assistant Duties

  • Prepare and room patients; take and document vital signs
  • Review medications and patient information; prepare exam rooms
  • Assist physicians during encounters; support scheduling and follow-up
  • Maintain infection-control and patient-safety practices

What You Bring

Required

  • Korean language proficiency
  • Strong willingness and ability to learn
  • Reliability, professionalism, and a strong work ethic
  • Sharp organizational skills and attention to detail
  • Ability to follow detailed clinical and administrative protocols
  • Comfort with electronic systems and new technology
  • Strong interpersonal and patient-service skills
  • Effective communication in English
  • Ability to rotate between assigned Queens practices
  • Commitment to HIPAA and patient confidentiality

Preferred, Not Required

  • MA certification or training
  • Prior patient-facing healthcare experience (primary care, internal medicine, geriatrics, or similar)
  • Experience with Medicare patients, AWVs, or preventive screenings (PHQ-9, cognitive assessments, spirometry, diabetic retinal screening)
  • Familiarity with EHR systems
