Who We Are

Five T Management is a values-driven property management and development company based on Chicago’s South Side. We develop, own, and manage affordable apartment homes that residents are proud to call home. Founded on the belief that the vibrancy of a community can be restored, we are committed to delivering high-quality housing while creating an engaged work culture where our team members can contribute, learn, and thrive.

We manage each property like it’s our own, stewarding both our residents’ living experience and our owners’ investment. Through our Family Self-Sufficiency program, we go beyond housing to help residents set goals, access resources, and build the economic independence they deserve. We proudly serve communities with care, excellence, and integrity.

About the Role

The Family Self-Sufficiency (FSS) Coordinator works directly with residents enrolled in the HUD-funded FSS program across Five T Management’s portfolio. This position helps each participant set meaningful one-year and five-year goals, connects them with supportive services, manages their escrow accounts, and tracks their progress from enrollment through graduation.

This role is primarily office-based with regular travel to properties across the Chicagoland area. Evening and weekend availability is occasionally required for resident events and community outreach. The ideal candidate combines strong case management skills with a genuine passion for economic mobility, and brings the discipline to keep compliance, documentation, and reporting consistently audit-ready.

Key Responsibilities

Resource Networking and Community Partnerships

  • Develop and lead the Program Coordinating Committee (PCC); host quarterly meetings to review program progress and gather community input.
  • Attend interagency meetings and maintain an up-to-date network of community resources and referral partners.
  • Connect residents with appropriate agencies and follow up to confirm successful referral outcomes.
  • Identify and promote local classes, workshops, and community events that support resident goal attainment.
  • Represent Five T Management at community events, speak on panels, and serve on committees as appropriate.

Program Operations and Enrollment

  • Develop and distribute program promotional materials; conduct outreach to drive enrollment across eligible properties.
  • Review applications, schedule and conduct eligibility interviews, and notify applicants of acceptance or denial.
  • Execute Contracts of Participation with new enrollees; establish one-year and five-year goals with each participant.
  • Prepare welcome packets and onboarding materials for new residents entering the program.
  • Assist with data entry and event coordination when housing lists open.
  • Share best practices with FSS coordinators across the broader network.

Resident Relations and Case Management

  • Meet regularly with participants to discuss goal progress, address challenges, and document case notes.
  • Provide and document referrals for social services including employment, education, job training, childcare, and financial empowerment resources.
  • Conduct occasional home visits to support participation and assess needs.
  • Coordinate and facilitate life skills development classes; produce and distribute a monthly calendar of offerings and track attendance.
  • Plan graduation ceremonies for program completers and produce press releases to celebrate resident success stories.

Escrow Management and Financial Tracking

  • Post and reconcile monthly escrow accruals to participant accounts; confirm accuracy and update tracking reports.
  • Provide residents with annual escrow statements showing accrual progress toward their goals.
  • Maintain escrow documentation in compliance with HUD FSS program requirements and 5T accounting standards.

Compliance and Reporting

  • Submit annual Performance Reports including updated Logic Model and program evaluation responses as required by HUD.
  • Prepare monthly activity reports and comply with all reporting requirements under Section 872 and applicable HUD guidelines.
  • Audit participant files and site documentation to ensure ongoing compliance with FSS program regulations.
  • Monitor participant compliance with program rules; take appropriate remedial action when residents are at risk of non-participation.
  • Keep the program and all associated properties in compliance with HUD FSS requirements at all times.

Qualifications

  • Associate’s degree in Business Administration, Sociology, or a closely related field required, plus a minimum of four years of experience in housing-related program administration; equivalent work experience will be considered.
  • Minimum of two years of experience in a supervisory or program leadership role.
  • Knowledge of HUD FSS regulations, including HUD 24 CFR Part 984, escrow account administration, and case management best practices.

  • Advanced proficiency in Microsoft Word, Excel, and Outlook; Yardi or similar property management software required.
  • Ability to develop SMART goals with residents and track progress against measurable outcomes.
  • Valid Illinois driver’s license required by date of employment and must be maintained throughout tenure.
