The Telehealth Technician/LPN provides prescribed medical treatment and personal care services to patients while collaborating with the PACT team. Responsibilities include managing telehealth modalities, performing routine nursing assessments, administering medications, and maintaining accurate electronic medical records.
Candidates must hold a diploma or certificate from an accredited practical or vocational nursing program and possess at least one year of experience in a primary or ambulatory care setting. A valid, unrestricted nursing license and current Basic Life Support and ACLS certifications are required.

As a Telehealth Technician/LPN, you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic, as directed by physician or mid-level provider. You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.
