The Care Manager is responsible for coordinating patient care and managing the progression of care. They are accountable for proactive discharge planning to ensure timely and safe patient transitions.
Candidates must be a graduate of a nursing program with a valid Georgia State or Multistate RN license and 2 years of acute or post-acute experience. A Master's degree and certification in Case or Care Management are preferred.
