The Case Manager RN coordinates comprehensive care and discharge planning for a specific patient population to ensure a safe transition through the continuum of care. They act as a resource for the healthcare team and patients while proactively resolving barriers to patient progression.
Candidates must hold a Bachelor's Degree in Nursing and possess a valid RN license in Pennsylvania or New Jersey. A minimum of 3 years of experience in case management or discharge planning is required.
