The Targeted Case Manager provides essential services within a CCBHC to help individuals sustain mental health and substance use recovery. The role focuses on connecting clients to medical, behavioral, social, and legal services to promote independence and community stability.
Candidates must have a bachelor's degree in social work, psychology, nursing, or human services, and at least one year of direct service experience with mental health or substance use disorders. Experience with CCBHCs or Federally Qualified Health Centers is preferred.

Position Description
The Full Time Exempt, Level 1 Targeted Case Manager reports to our Director of Care Management.The Level I Targeted Case Manager provides targeted case management services within a Certified Community Behavioral Health Clinic (CCBHC). This role assists individuals in sustaining mental health and substance use recovery and gaining access to needed medical, behavioral health, social, educational, legal, and community services. The targeted case manager focuses on connecting individuals to services that help them resolve problems interfering with their independence or self-sufficiency and maintain themselves in the community.
Our standard hours are Mon-Fri, 8:30am-5:00pm.
