Provide primary care services to geriatric patients within skilled nursing, assisted living, and independent living facilities. Focus on delivering excellent patient care while actively working to reduce hospital readmissions.
Must be a qualified Physician capable of practicing primary care in a geriatric setting. Ability to serve homebound patients and facility residents across multiple counties in Central Florida is required.
Our company was founded in 2005 and serves the Central Florida geriatric population. We are a large practice with over 25 practitioners that serve the skilled nursing facilities, assisted living facilities and our homebound patients in the comfort of their home in 14 counties. We are committed to providing excellent care to our patients and buildings while reducing readmissions into the hospital.
