We are currently seeking qualified candidates for the following position:

The Case Manager is a key provider of services within Volunteers of America’s Case Management Model. The Case Manager works directly with clients to assess their strengths and needs, develop and implement a service plan, link the client to appropriate resources, counsel the client, and monitor progress. The Case Manager ensures that clients receive timely, professional, and comprehensive support to achieve successful outcomes while maintaining a strong focus on client engagement and service access.

 

Supportive Services for Veteran Families (SSVF) is a community-based outreach program, we identify low-income veteran households and connect them with a range of supportive services to promote housing stability and prevent homelessness. Location: Bronx, NY 10451

 

Minimum Qualifications:

  • 60 college credits plus two years relevant experience, or satisfactory combination of education and experience.
  • Demonstrated ability in verbal and written communication, including basic computer skills.

 

Case Manager Principal Responsibilities:

  • Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
  • Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings goals. Update the plan with the client as required.
  • Responsible for conducting outreach to veterans enrolled in the Supportive Services for Veteran Families (SSVF) program, ensuring their active participation and access to services, this include home visits or conducting outreach in shelter settings as necessary.
  • Meet regularly with the client to assist him/her in obtaining needed services/benefits to achieve their individual service plan goals. Provide supportive counseling to strengthen his/her ability to make appropriate life decisions.
  • Document case management activities in accordance with agency and funder guidelines and procedures. Collect service data and participate in evaluation and quality assurance activities.
  • Support outreach with screening and assessment efforts to identify veterans who may be eligible for SSVF services.
