Population Health Specialist

General Summary:

A Population Health Specialist assists with the development and implementation of population health strategies and initiatives, providing coordination and support for the population health team. This role assists in managing projects including communications, special programming, and other initiatives.

Primary Responsibilities:

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification. They are not to be construed as an exhaustive list of all job responsibilities performed.

1. Plans, implements, and monitors population health programs that support the mission, values, goals, and objectives of the system and the local site as directed by manager or director.
2. Assists with writing and/or purchasing collateral material to support population health initiatives.
3. Represents local site and/or UMMS to local community partners to build relationships and collaborate on programing.
4. Works on planning and development of projects with a variety of internal and external partners.
5. Assists in the development and implementation of population health programs for targeted populations.
6. Perform all other duties as assigned.

Knowledge, Skills, & Abilities:

1. Ability to manage, develop, plan, and deliver projects with minimal direction while meeting deadlines.
2. Working knowledge of computer applications with experience in word processing and spreadsheet software such as Microsoft Word, PowerPoint, Excel, and CBISA.
3. Excellent communication, interpersonal, and organizational skills.
4. Ability to work effectively with the public, community partners, vendors, and professional staff.
5. Cultural competence to understand the various backgrounds of the individuals served.
6. Ability to organize and analyze data on a computer.
7. Empathy: health workers must have empathy to best serve their community
8. Understanding of clinic operations, both inside and outside of strict healthcare settings.
9. Weekend, shift work, holiday, on-call, may be required due to community related work.
10. Work is often performed outside of the hospital in various locations respective to the hospital service areas. Locations may include local churches, schools, senior centers, parks, mobile van, or business offices.
11. Local travel required.

Education & Experience - Required:

1. High school diploma or GED is required.
2. One (1) to Two (2) years of relevant experience is required.

Education & Experience - Preferred:

1. Bachelor’s degree is preferred.
2. Two (2) to five (5) years of related experience preferred.
3. Current CPR experience and/or Emergency First Aid training preferred.
