What You’ll Do:
- Evaluate patients and provide personalized speech therapy services that improve communication, cognition, swallowing function and overall quality of life
- Develop and implement patient-centered treatment plans focused on speech, language, cognitive and swallowing rehabilitation goals
- Deliver evidence-based therapy interventions that support functional independence in adult and geriatric patients
- Partner with interdisciplinary teams, caregivers, and nursing staff to support strong patient outcomes and continuity of care
- Maintain accurate clinical documentation, monitor patient progress and adjust treatment plans based on individual rehabilitation needs
Qualifications
Qualifications:
- Licensed Speech Language Pathology (SLP) or license eligible in the state of practice
- Graduate of an accredited Speech Language Pathologist Program
- Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
- Passion for patient-centered care and improving quality of life for older adults
- Willingness to travel to multiple TMC locations as needed
