Responsibilities

What You’ll Do:

  • Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence
  • Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
  • Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults
  • Help patients restore functional mobility and safely perform daily activities with greater confidence and independence
  • Quickly integrate with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes throughout your assignment
  • Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications

Qualifications:

  • Licensed Physical Therapist (PT) or license eligible in the state of practice
  • Graduate of an accredited Physical Therapist Program
  • Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
  • Passion for patient-centered care and improving quality of life for older adults
  • Willingness to travel to multiple TMC locations as needed
