About the Program

This is an exciting opportunity to be a part of Metro-Boston’s PACT-Y team through the Home for Little Wanderers.  PACT (Program for Assertive Community Treatment) is an evidence-based home/community centered model.

The PACT-Y Outreach/Case Manager provides intensive, community-based support to youth with significant emotional and behavioral health needs as part of a multidisciplinary team. This role focuses on engaging youth and families in services, supporting individualized treatment goals, and coordinating care across systems.
 
This is a highly collaborative, field-based position that requires flexibility, strong relationship-building skills, and the ability to adapt to the evolving needs of youth and families.
 
Key Responsibilities
  • Deliver community-based outreach and case management services in home, school, and other natural settings
  • Engage youth and caregivers to build consistent, trusting relationships
  • Support implementation of individualized treatment plans in partnership with the team
  • Assist with crisis prevention and intervention, utilizing a team-based approach
  • Coordinate care with providers, including mental health, medical, educational, and state agencies
  • Support development of life skills, emotional regulation, and social functioning
  • Connect youth and families to community resources and supports
  • Participate in daily team meetings and ongoing treatment planning
  • Complete timely and accurate documentation in compliance with program standards
  • Provide transportation support for youth as needed
  • Schedule: Full-time-with flexible hours including evenings and occasional weekends.

Qualifications

  • High School diploma or GED. Bachelor’s degree preferred.
  • Must be able to work with people of different backgrounds, languages and ethnicities.
  • Driver’s license required.

Actual compensation will be based on a variety of factors, including a candidate’s relevant experience, education, skills, and internal equity relative to other employees in similar roles. Candidates with less directly related experience may be placed toward the lower end of the range, while those with extensive or specialized experience may be placed toward the higher end. Market conditions and budget considerations may also influence final pay decisions.
