Clinical Pharmacist

Work hours: Full Time (40 hours) Rotating Shifts
The candidate will be responsible for monitoring and verifying medication orders for wholly owned acute care hospitals and Texas Health Recovery and Wellness center. Candidate will also be supporting the ongoing expansion of discharge medication reconciliation process. This role also participates in many ad hoc projects to support system pharmacy operations and quality improvement efforts.

Here’s What You Need
Education
· Bachelor's Degree Pharmacy required or
· Doctor of Pharmacy degree preferred and
· Other - CLIAFE Must comply with CLIA regulations regarding foreign education equivalency required
Experience
· 5 Years Experience as a hospital pharmacist with expertise or training in specialty area preferred
· Experience with CareConnect 1 (epic) system strongly preferred.
Licenses and Certifications
· PHAR - Pharmacist upon hire required
What You Will Do
· Communicating effectively, building positive professional relationships and role modeling teamwork.
· Timely and accurate support of medication distribution including medication order verification
· Provision of clinical pharmacy services for the safe and efficient patient care.
· Ensuring the accuracy of one's own work as well as maintaining required competencies and other documentation. Consistent support of the department's quality improvement and patient safety standards programs and initiatives is also required.
· Ongoing professional development and growth.
· Not only the management of one's own time but shares the responsibility for minimizing pharmaceutical costs (including wastage) and discovering new process efficiencies.
· Other duties as assigned.
