Why Telepatia

Telepatia builds real-time AI products for healthcare — medical transcription, clinical record generation, and EMR integrations across Brazil and Colombia. Our integrations layer is the nervous system connecting hospitals, clinics, and EHR systems to our platform.

We need someone who can design, build, and own complex integration systems where reliability directly impacts clinical workflows. This person won't just execute tasks — they'll shape how the team thinks and builds. Reliability here has real-world consequences for clinical teams.

What You'll Do

  • Map, don't pack — Look at complex, undocumented systems and build a clear mental model from first principles, not just from specs handed to you

  • Move with high agency — Don't wait to be unblocked. Hit a wall? Find a way around it, document the decision, and keep the team informed

  • Own production — Write runbooks, follow up on incidents, ship and stand behind your work

  • Mentor actively — Raise the floor of the team through meaningful code reviews and explaining the why, not just the what

  • Engage with open source — Read source code to debug issues, contribute patches, and build thoughtfully on top of OSS

    What You Have

  • Experience 5+ years of hands-on backend engineering experience

  • Must-have skills Node.js · TypeScript · Python · REST API design · GraphQL API design · PostgreSQL · MongoDB · Strong backend fundamentals (queues, async patterns, error handling, observability)

  • Good to have Kubernetes · GCP · Redis / BullMQ · Docker · ArgoCD

  • Nice to have Pulumi / IaC · Linkerd / service mesh · Kafka / Pub-Sub · Firestore

  • Fullstack signal Can read and reason about React/Next.js code; designs APIs with the consumer's DX in mind, not just correctness

  • How you work Comfortable with ambiguity · Direct communicator · Intellectually curious about the Node.js and Python ecosystems · Ego-free on code — pushes back through good arguments, not attachment
