Hospice RN Case Manager

Mason City

$10,000 sign on bonus

Position Summary
The Hospice RN Case Manager provides compassionate, patient-centered nursing care to hospice patients and families. This role is responsible for assessment, care planning, symptom management, coordination of services, patient and caregiver education, and overall case management to ensure quality end-of-life care.

Key Responsibilities

  • Manage a hospice patient caseload and serve as primary case manager.
  • Complete comprehensive patient assessments and develop individualized care plans.
  • Coordinate care with physicians, hospice aides, LPNs, social workers, chaplains, and other interdisciplinary team members.
  • Manage pain and symptoms using evidence-based hospice and palliative care practices.
  • Educate patients and families on disease progression, end-of-life care, and available hospice services.
  • Maintain accurate and timely clinical documentation.
  • Participate in interdisciplinary team meetings and care conferences.
  • Supervise Hospice Aides and LPNs, including required supervisory visits and care plan oversight.
  • Support patients and families through grief, loss, and emotional transitions.

Qualifications

  • Graduate of an accredited nursing program.
  • Current RN license in the state of practice.
  • CPR certification required.
  • Minimum one year of acute care nursing experience required.
  • Hospice or home health experience preferred.
  • Strong communication, assessment, and relationship-building skills.
  • Valid driver's license and reliable transportation.
