Description:

Directs daily operations of the case management department to ensure constant and efficient flow of patient care. The Team Leader works in collaboration with a multidisciplinary team promoting the values, mission, vision, and goals of the hospital to facilitate patient transitions efficiently and in a cost-effective manner that ensures quality outcomes. The Team Leader will oversee daily staffing needs to ensure safe and efficient patient care. The Team Leader provides an “administrative” presence in the absence of the Director/Nurse Manager, and they are responsible for holding staff accountable to policy/procedure/department rules.

St. Mary's Healthcare System is seeking a highly motivated Case Management Team Leader to provide daily operational leadership for the Case Management department. This role is responsible for ensuring the efficient flow of patient care services, supporting safe and effective patient transitions, and promoting positive patient outcomes through collaboration with a multidisciplinary healthcare team.

Key Responsibilities:

  • Direct and coordinate daily operations of the Case Management department.
  • Oversee daily staffing assignments to ensure safe, efficient, and effective patient care.
  • Collaborate with physicians, nurses, and interdisciplinary teams to facilitate timely and cost-effective patient transitions.
  • Promote and uphold the mission, vision, values, and goals of St. Mary's Healthcare System.
  • Serve as the department's administrative leader in the absence of the Director or Nurse Manager.
  • Hold staff accountable to organizational policies, procedures, regulatory requirements, and departmental expectations.
  • Support staff development, engagement, and operational excellence initiatives.

Qualifications:

  • Registered Nurse (RN) or Master Social Worker (MSW) with experience in Case Management.
  • Demonstrated leadership experience in an acute care hospital setting preferred.
  • Strong communication, organizational, problem-solving, and team-building skills.
  • Knowledge of utilization management, discharge planning, care coordination, and regulatory requirements.
