Role Overview

Family Connections (FC) Case Planner provides evidence-based intervention for youth and their families. This high-quality, strength-focused family counseling model is designed primarily for at-risk families who have been referred by ACS or their community. Services are conducted in home settings and can also be provided in schools and child welfare facilities. Flexible schedule including some nights and weekends, depending on the needs of the client.


Key Responsibilities

  • Participates in interviewing and hiring of Family Childcare Providers. 
  • Facilitate ongoing in-home family sessions, with families in Manhattan and the Bronx.
  • Engage all family members in active change focused intervention through facilitation of the Family Connections model.
  • Conduct ongoing risk and safety assessments, utilizing identified familial strengths to address risk and safety factors.
  • Maintain clear and concise documentation, meeting both model specific and ACS requirements.
  • Participate in all model specific trainings, ACS trainings, individual and group supervisions, and ongoing program and department meetings and events.
  • Collaborate with ACS partners
  • Other related duties as assigned by supervisor.

Required Qualifications

  • Bachelor’s degree required
  • Good written and oral communication skills; computer literate.
  • Experience in child welfare field strongly preferred. 
  • Excellent time management skills with the ability to meet deadlines in a timely manner while managing competing priorities. 
  • Bilingual preferred, (preferably Spanish, French/Creole) to conduct case management for our families. 
  • Proficient use of Microsoft applications; knowledge of PROMIS/CNNX desired. 
  • Cultural awareness and sensitivity when serving clients that come from different backgrounds and walks of life.

Position Type/Expected Hours of Work

This is a full- time position. Days and hours of work are Monday through Friday Hybrid at our Queens Hub and Far Rockaway Hub site location. Work hours options are 8:00 am - 4:00 pm, 8:30 am - 4:30 pm, 9:00 am - 5:00 pm, 10:00 am - 6:00 pm
