We are hiring a Medical Social Workday (MSW) for a Home Health position providing skilled nursing care to patients in their homes in the Parkesburg, PA area.
What You’ll Do:
Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
Conduct comprehensive psychosocial assessments, identify patient and family needs, and develop individualized care plans that address emotional, social, and environmental factors
Educate and support patients and families by connecting them with community resources, counseling services, and care planning guidance to ensure they feel informed, supported, and empowered
Apply critical thinking and problem-solving skills to navigate complex situations, advocate for patient needs, and maintain accurate, timely, and compliant EMR documentationWork independently, but never alone
Travel to patient homes within assigned territory

MSW Qualifications
Master’s in Social Work from graduate school accredited by the Council of Social Work
One (1) to two (2) years post graduate social work experience in a health care setting
Licensed MSW preferred
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
