We are hiring a Certified Nursing Assistant for a Home Health position providing care to patients in their homes in the Pottsville, PA area.

What You’ll Do:

  • Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
  • Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
  • Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
  • Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
  • Work independently, but never alone
  • Travel to patient homes within assigned territory

CNA Qualifications

  • Active Certified Nursing Assistant (CNA) certificate in the state you work
  • High School Diploma or certificate
  • Minimum one year healthcare experience
  • Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
