About this role

Definition:
Provide case management for Renewal reentrants with mental health and/or drug and alcohol diagnosis/histories.  Will act as the primary contact for reentrants and assist the reentrant with connecting them with clinical services.  The primary responsibility will be to assist the reentrants with managing their medication, treatment and community schedules.  The Mental Health Case Managers will assist the reentrant with creating and implementing a case management plan.  Must enforce compliance to program guidelines and assist the Mental Health Case Management Supervisor in the functioning of work release center's daily activities (administration), in initiating and developing policies and procedures, and maintaining records.

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and/or ability required.

Education and/or Experience:
Knowledge of Criminal Justice, Social Work, Psychology, Counseling, Public Policy, or other related field; human relations skills a must; should become familiar with the Criminal Justice/MH systems and community resources.  Two years experience providing mental health and substance abuse services.  Demonstrated ability to work effectively and efficiently with all levels of management, employees, and reentrants; ability to deal effectively with reentrants and referral sources; self-starting, motivated, and adept to changing technology; proven skills at problem solving, conducting research; should be computer literate in word processing, spreadsheets and databases.  Ability to communicate and to help the reentrants express their own needs and aspirations.

Bachelor's degree in Criminal Justice, Psychology, Counseling, Social Work, and a minimum two years experience in related field required.
