Job Responsibilities
- Delivery of safe care and sound clinical decision making
- Assessing and documenting observations of clients condition at each visit
- Initiating/ reviewing nursing care plan based on assessment made
- Maintaining accurate nursing documentation
- Performing necessary nursing procedures such as wound dressing, urinary catheter insertion, nasogastric tube insertion, medication packing and etc.
- Conducting regular evaluation of care provided for home care services
- Provide caregiver training and education to client including families on home health care procedures and strategies
- Reporting of any problem incidents encountered in a client’s condition and/or in handling clients and/or caregiver to Nurse Lead
- Participate in project work to improve the quality of care and services to patients
- Identify service gaps in response to clients’ needs and make recommendations to improve service provision of home care
- Participate in integrated Home Health project to provide palliative care services for home health clients which includes telesupport
Job Requirements
- Diploma in Nursing
- 2 years of experience in community nursing/ home care
- Those with palliative care / oncology training or background will be an added advantage
- Capable of performing a wide range of complex nursing procedures without immediate on-site supervision
- Ability to work independently with strong decision-making skills in a home setting
- A positive, empathetic personality with a genuine passion for client care
- Proficient in MS Office applications
