Our 62-bed Behavioral Health Unit is seeking compassionate mental health clinicians to care for patients with psychiatric illnesses.  Our recovery-oriented program provides opportunities for professional growth in a collaborative care environment.

 

  • 62-bed Inpatient Psychiatric Center
  • Adult Psych Unit (ages 18- 64)
  • Geriatric Psych Unit (ages 65 and older.) (Patients in this unit requires assistance with all aspects of daily living -Total Care)
  • Intensive Outpatient Program
  • ECT (Electroconvulsive Therapy)
  • Social Workers are expected to conduct group therapy, psychosocial assessments, and discharge planning.

 

Responsibilities

KEY RESPONSIBILITIES:1. Assesses, evaluates, plans and coordinates community services2. Advocates for services to meet the specific patients / family complex needs3. Conducts high risk assessments within timeline required by departmental and regulatory guidelines4. Coordinates with Care Manager daily5. Conducts psychosocial assessments6. Provides patients / family members with Community Resources7. Coordinates the transition to post-acute care services (Hospice, Home Health, Skilled Nursing Facility, etc.)8. Coordinates as needed with other members of the care team9. Organizes family meetings and/ team conferences10. Works with the treatment team to provide solutions for complex cases (i.e. Behavioral Health and/or barriers to discharge)11. Identifies high risk patients based on standardized criteria12. Coordinates appropriate reporting to legal agencies as needed with respect to abuse and neglect13. Facilitates the coordination of financial assistance as needed14. Identifies and documents quality variances and/or barriers to discharge15. Provides post discharge follow-up as appropriate to ensure continuity of care/services16. Participates in PHC readmission management initiatives

  • Master’s degree from an accredited social work program Required

  • LMSW - Licensed Medical Social Worker - State Licensure in the state of Georgia Preferred
