JOB SUMMARY:
To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. Registered Nurses will provide 24/7 on site coverage of assigned residential program/s. The Nurse is responsible for oversight and provision of medication administration and medication monitoring; oversight and coordination of physical health of residents; provision of metabolic screenings; vitals; restraint oversight and follow up assessments, and collaboration with residential team including psychiatrist.
PHYSICAL DEMANDS:
Minimal in nature, ability to perform CPR and approved physical interventions after successful completion of training.
QUALIFICATIONS
At minimum, successful completion of RN program and active PA RN license in good standing; Experience in behavior health care; RN license; Valid Driver's License and use of personal vehicle; Excellent interpersonal skills
SPECIFIC DUTIES:
- Be knowledgeable and comply with DPW’s 55 PA Code Chapter 3800 (Child Residential and Day Treatment Facilities Regulations), OMHSAS’s memo date November 1995: Requirements for Residential Treatment Facilities for Adults, and 6400 regulations.
- Be knowledgeable and comply with CARF, CBH, PRTF and DHS requirements/regulations.
- Provide and ensure all aspects of Medication Administration and Medication Monitoring, to include trainings, coordination with pharmacy and collaboration with psychiatrist/s are completed in a timely, comprehensive manner.
- Complete Metabolic Screenings and other assessments/screenings (e.g. AIMS) on each resident.
- Provide oversight of physical health needs for each resident, including ensuring any dietary needs are met.
- Complete health histories, as assigned.
- Provide vitals and medical intervention, as indicated.
- When indicated, coordinate PCP, Urgent Care or Emergency Room visits.
- Obtain orders for restraints, oversee restraints and provide follow-up assessments after a restraint.
- Collaborate with the psychiatrist/s on any medication and/or evaluation needs
