Case Manager Assistant
About this role
Outsourcing Staffing Advantage is seeking a Case Manager Assistant who will serve as an important point of contact for patients, families, physicians, healthcare providers, and service disciplines. This role will assist with day-to-day case coordination, communication, documentation, service orders, and follow-up to ensure patients receive the appropriate services in a timely and organized manner.
The successful candidate will work closely with an RN Case Manager and will receive comprehensive training on the company's case management processes, medical record system, service coordination procedures, and documentation requirements.
Key Responsibilities
- Assist the RN Case Manager with the day-to-day coordination and management of assigned patient cases.
- Maintain regular communication with patients and their family members regarding services and care coordination.
- Communicate professionally with physicians, medical offices, healthcare providers, and other service disciplines involved in patient care.
- Coordinate communication between patients, families, physicians, and service providers to ensure continuity of care.
- Create and process orders for services within the company's medical record system.
- Document patient communications, follow-ups, service coordination, and other case-related activities accurately and promptly.
- Maintain organized and up-to-date patient records and case documentation.
- Follow up with providers and other parties to ensure requested services and documentation are completed appropriately.
- Assist with coordinating services based on patient needs, insurance coverage, authorizations, and established care plans.
- Escalate questions, concerns, or complex case issues to the assigned RN Case Manager.
- Work collaboratively with the RN Case Manager to resolve service coordination issues and ensure appropriate follow-up.
- Monitor assigned cases and communicate any changes, delays, concerns, or barriers that may affect the patient's services.
- Follow company procedures, documentation standards, and compliance requirements.
- Participate in ongoing training and coaching provided by the company's clinical and case management team.
- Maintain professional communication and confidentiality when handling patient information.
- C1 English and Spanish proficiency.
- Previous experience in healthcare, medical administration, medical billing, insurance, patient services, or a related field is strongly preferred.
- Basic understanding of the U.S. healthcare system.
- Familiarity with Medicare and Medicaid is highly preferred.
- Basic knowledge of health insurance coverage, eligibility, authorizations, and covered services.
- Experience with medical records, documentation, or healthcare-related administrative systems is a plus.
- Strong organizational and follow-up skills.
- Excellent attention to detail and accuracy.
- Ability to manage multiple patients, tasks, communications, and deadlines simultaneously.
- Strong interpersonal and professional communication skills.
- Ability to communicate clearly and professionally with patients, family members, physicians, and healthcare providers.
- Ability to learn new healthcare systems, processes, and procedures quickly.
- Comfortable working independently while knowing when to escalate questions or concerns to the RN Case Manager.
- Must be comfortable working remotely and collaborating through Microsoft Teams and other communication platforms.
