- Recruits and enrolls families into the Home Visiting programs by a variety of engagement methods including telephone, texting, letter writing and home visits
- Maintains enrollment throughout the program year through various recruitment strategies; and ensures vacancies are filled immediately
- Responsible for maintaining program case load as determined by the grant requirements
- Maintains a caseload according to program model and/or funder specific standards, providing home visits, to families specific to program and/or funder requirements
- Establishes trusting, nurturing relationships with parents and their children by providing family support services in the family’s home
- Ability to provide crisis intervention services to families based upon need
- Organizes and facilitates parent groups including prenatal groups per grant requirements
- Works jointly with parents to develop and implement home visit plans based on identified family and child strengths and needs
- Provides evidence/research-based information to families on a variety of topics including but not limited to home safety, child development, and the importance of medical and dental care
- Observes, assesses, and documents each child’s health, skills, behavior, growth, and developmental milestones
- Provides children and parents with group Socialization experiences per program requirements
- Provides program parents with guidance on home safety, nutrition, effective discipline, constructive play activities, basic child care, crisis management, problem solving, positive relationships and other related topics.
- Integrates other components (i.e. health, nutrition, mental health, parent involvement, and special needs) into home visit and services provided
- Assists in identifying children with special needs and makes the necessary referrals
- Encourages family participation in the program specific Advisory Committee meetings as well as prenatal and/or parent group, and other meetings, events and/or trainings
- Assists families in developing a Family Goal Plan which includes a plan for the accomplishment of the goals
- Completes all required program/model specific trainings no later than 6 months of hire
- Completes all required supplemental training per model/funder guidelines and timeframes
- Ongoing and additional training required including but not limited to working with highly stressed families in overburdened communities, issues related to maternal/child health, child abuse, neglect and maltreatment, labor and birth, inductions, cesarean sections, postpartum period/newborn care, breastfeeding, childbirth education, child development of infants and toddlers, CBFANA and group facilitation
- Willingness to register, complete the pre-assessment, and receive training within the Institute for the Advancement of Family Support Professionals and certify with the National Family Support Certification within five years of hire
- A desire to become familiar with infant mental health practice by attending regular team meetings and trainings with the I/ECMH consultant
- Become certified in CPR, Infant CPR and First Aid within the first year of hire
- Collaborates with program supervisor and other agency staff to review and maintain home visiting model affiliation and/or agency accreditation standards including COA Accreditation standards
- Develops and maintains Community Partnership Agreements (MOU)
- Remains aware of community resources to support the needs of families and makes appropriate referrals
- Partners with co-workers to maintain program resource file within the office
- Attends Early Intervention meetings with the families receiving services in order to support developmental and/or social needs of child and family
- Assists with attending community networking, community outreach and marketing efforts as required, targeting local community based resources, including but not limited to food banks, mental health programs, domestic violence resources,
- Assists and participates in fundraising activities as needed
- Maintains and preserves confidential client files Understands the program’s outcome goals and works diligently with the families to pursue positive outcome goals while participating in services.
- Familiarizes and complies with all policies and procedures of the agency, funders, and accreditation bodies
- Follows established guidelines for use and/or disclosure of protected health information; reports any breaches of the Health Insurance Portability and Accountability Act (HIPAA) rules to their supervisor immediately
- Organizes and maintains complete, accurate, and up-to-date records for children and families according to program and/funder requirements including data entry into various databases such as agency, funder and state. Information to include but not limited to demographics, social and child development screenings, home visit records, goals, outcomes,
- Documentation of case records to be completed within the required time per program
- Maintain accurate account of daily activities through Outlook scheduler, reporting any changes of schedule to supervisor
- Participate in regularly scheduled program team meetings, doula clinical meetings, infant mental health meetings and other agency meetings
- Participate in regularly scheduled reflective supervision with program supervisor or designee
- Maintain accurate documentation of time utilizing Paycor or another agency program, submitting every two weeks
- Maintain accurate documentation of expenses including mileage utilizing Ramp or another agency program
- Documentation of required program monthly reporting using an End of Month Report or other specified reporting method
- Willing to work flexible hours, some weekday evenings and weekend (Saturday) hours according to the needs of the program and families which include but are not limited to group connections, end of year and holiday celebrations, and home visits.

Education
- High School Diploma required
- Bachelor’s Degree in human services related field such as childcare, early childhood development, education, social services, nursing, or equivalent work experience with families and children (as required by program and funder) preferred
- Two years' previous supervised work experience working with or providing services to young children and/or parents/families; ideally in home visiting, child welfare, residential care of community-based setting required.
- Knowledge of Lake County resources preferred
