Job Description
We are seeking a dedicated and experienced Intact Case Manager to join our team. The Intact Case Manager will be responsible for providing comprehensive case management services to families involved in the child welfare system. This includes conducting assessments, developing service plans, coordinating services, and monitoring progress towards goals.
Responsibilities:
- Conduct initial assessments to determine family needs and strengths
- Develop individualized service plans in collaboration with families and other service providers
- Coordinate and monitor services to ensure that families are receiving appropriate support
- Provide crisis intervention and support to families as needed
- Maintain accurate and up-to-date case records
- Participate in case conferences and team meetings
- Collaborate with community partners to access resources and support for families
Qualifications:
- Bachelor's degree in social work, psychology, or related field
- Previous experience working with children and families in a social services setting
- Knowledge of child welfare laws and regulations
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
