To assist in achieving this mission, NAC is currently seeking a Case Associate to join its Special Medical Prevention Program.

Our Prevention Program delivers a range of clinical, case management, and supportive services to families who are child-welfare involved and caring for children diagnosed with chronic or complex medical conditions.
