Location: Mostly Remote (Includes Local Home Visits)
Schedule: Monday – Friday, 8:30 AM – 4:30 PM (plus rotating on-call schedule)

What You’ll Do

Coordinate & Support: Receive new referrals, manage intakes, and create personalized service plans for home and community-based services.

Connect in the Field: Conduct regular home visits to ensure direct care quality and support standards are met.

Build Relationships: Partner closely with clients, case managers, direct care workers, and community partners.

Maintain Operations: Oversee scheduling adjustments, handle incident investigations, and provide rotating after-hours on-call support.
What You Bring

Education & Experience: High school diploma or equivalent required.

Proven Track Record: Minimum 3 years in service coordination/delivery and 2 years in a supervisory role.

Certifications: CPR/First Aid, Article 9, and Principles of Caregiver certification (or Instructor status).

Core Skills: Strong organizational skills, computer proficiency, accurate record-keeping, and problem-solving ability.

Language Skills: Spanish or Russian bilingual skills are highly desirable (preferred)!
