SUMMARY OF RESPONSIBILITIES:

The Discharge Planner is responsible for coordinating the discharge process for all CSU clients, post discharge follow-up, maintaining cooperative relations with community agencies and service providers, and meeting with families to develop and implement appropriate discharge plans for after care

 

MINIMUM QUALIFICATIONS:

This position requires a Bachelor’s Degree in a Behavioral Healthcare field with a minimum of 2 years of mental health related experience.  Master’s Degree preferred. 

 

 ESSENTIAL DUTIES/RESPONSIBILITES: 

  • Provides and documents timely and thoroughly treatment services, assessments, interventions, and aftercare plans. 
  • Facilitates the exploration of continuing care issues that include: returning home, post program placement, participation in self-help groups, partial hospitalization, and intensive out-patient programs.
  • Arranges appropriate post discharge referrals
  •  Engages with collateral contacts and providers regarding client’s discharge needs 
  • Participates in treatment teams to provide collateral information for discharge planning
  • Enters information into CFBHN’s SharePoint system as required.
  • Computer Literacy
  • Completes all training requirements in a timely manner 
  • Completes all other duties and special projects as assigned
