We are recruiting to a new post within the Children and Family Wellbeing Service. The role of the SEND Support Facilitator is part of the expansion of the CFW SEND Team.

The post holder will work collaboratively with teams across the Children and Family Wellbeing Service, Commissioned Providers, Parent/ Carers and the wider Early Help Partnership to ensure that local support is accessible, inclusive and achieves the best possible outcomes for SEND children and Young People.

This vacancy is a permanent post based in Lancaster 

In the role of the SEND Support Facilitator you will provide specialist advice, support and consultation to SEND Children, Young People and their families, who are supported by our Family Intensive Support service. You will provide consultation to our team members and partners who work across the Children and Family Wellbeing service and the Early Help Partnership.  The SEND Support Facilitator will offer support and signposting for SEND Children, Young People & Families who access our Family Hubs and wider service offer.


As a SEND Support Facilitator you will support with the development and delivery of inclusive Family Hubs and inclusive group work provision for SEND Children & Young People. You will look to liaise with wider SEND services and consider opportunities for service development and co delivery.  You will look to develop strong links with the Lancashire Parent Carer Forum and our Family Hub Parent Carer Champions to facilitate the co design of services and support within our service.  


This is a new role within our service and will provide you with the opportunity for creativity, development and growth as the role develops and the offer is embedded within our service delivery. The CFW SEND team is growing, alongside our ambition to provide quality, needs led support and services.
