Position Summary
The Physical Therapist is responsible for evaluating, planning, and implementing individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical care team.
This role supports patients in achieving greater independence, mobility, and quality of life through evidence-based therapy delivered in the home setting.
Key Responsibilities
Deliver effective patient care using approved physical therapy techniques and clinical standards
Perform comprehensive evaluations and establish measurable treatment goals
Develop and implement individualized treatment plans
Supervise and coordinate services with Physical Therapist Assistants (PTAs) and Home Health Aides (HHAs)
Complete timely and accurate documentation of patient progress and treatment plans
Educate patients and caregivers on therapeutic exercises, mobility strategies, and adaptive equipment
Assess home environments and recommend modifications to improve safety and functional independence
Maintain HIPAA compliance and patient confidentiality standards
Collaborate with nursing, social work, and interdisciplinary team members to ensure comprehensive patient care
Manage caseload and productivity expectations to meet 30 points per week
QualificationsQualifications
Active California Physical Therapist (PT) license
Minimum 1 year of Physical Therapy experience preferred
Physical examination completed within the last 6 months
Current TB test (within the last 12 months)
Current CPR certification
Valid driver’s license and reliable transportation
Home Health experience preferred but not required
