Responsibilities:
- Coordinate caregiver schedules and client assignments
- Assist with onboarding and training new caregivers
- Coordinate client admissions and nursing assessments
- Develop and maintain care plans
- Monitor WellSky documentation and compliance
- Maintain accurate client and caregiver records
- Communicate with caregivers, nurses, clients, and family members
- Follow up on missed visits and scheduling changes
- Ensure quality care and client satisfaction
- Maintain HIPAA compliance
Qualifications:
- High School Diploma or GED required
- Associate's or Bachelor's degree in Healthcare Administration, Nursing, or related field preferred
- Home care, healthcare, or care coordination experience preferred
- Strong organizational and communication skills
- Proficient with Microsoft Office and Outlook
- Valid driver's license and reliable transportation
