Music Therapist                              

 

What You Must Have: 

  • Bachelor’s degree or equivalency in Must Therapy. Masters’ degree preferred.
  • Licensed to practice as a Mustic Therapist – Board Certified (MT-BC) within the state.
  • Demonstrates exceptional verbal and written communication, and organization skills.
  • Understands hospice philosophy, focus of care is comfort vs. rehab and to maintain care in their residence as long as possible.
  • Must have a valid driver’s license for the state providing service within.

 

What You Will Do:

  • Assesses music therapy needs of the patient/family and provides music therapy to the hospice patients and their families, as appropriate.
  • Consults with interdisciplinary team about patient referrals to assess needs and prioritize intervention.
  • Document all care provided on the patient’s clinical records in accordance with documentation requirement
  • Educate hospice staff and community about the role of Music Therapist and the use of music therapy interventions with hospice patients and family.
  • Provide bereavement follow-up to family members when determined appropriate by the interdisciplinary team.
  • Adhere to the practice of confidentiality regarding patients, families, staff and the organization
  • Serve as a music enhancement resource to staff
  • Support and promote Hospice’s mission to lead in providing exceptional patient – centered end of life care
  • Participate in Hospice’s success by ensuring access to our exceptional care
  • Timely submission of all documentation
  • Performance of other duties as required
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
