Outreach Specialist

What You'll Do
The Outreach Specialist is responsible for reaching out to participants before scheduled appointments as well as engaging or reengaging participants who fail to report to program-scheduled appointments and services.
Your Responsibilities Will Include:
1. Participant Intervention
Reach out to participants who have scheduled appointments to confirm that participant is aware of the appointment and/or if they need to reschedule.
Conduct escalating outreach to clients who fail to report to mandatory appointments and services. This includes telephone, email, mail, and home visit outreach strategies.
Identify and assist with mitigating barriers to program participation, including but not necessarily limited to, health, mental health, reasonable accommodations, housing, childcare, domestic violence issues.
Re-engage clients by discussing the range of available services, including reasonable or travel accommodations, and the potential benefits of participation in WeCARE.
Interdepartmental Coordination
Communicate to appropriate staff any informal accommodations provided to participants while in outreach.
Case conference with WeCARE Clinical Assessment, Case Management and Vocational Rehabilitation Service (VRS) staff, as necessary.
Work closely with VRS timekeeping unit team to ensure maximized interaction and engagements with all WeCARE.
Record-keeping and Compliance
Post the necessary case notes of outreach services and upload outreach documentation into HRA and Fedcap management systems, including but not limited to, SEAMS and FedcapCARES.
Submit all records of escalating outreach interventions determined by WeCARE guidelines into HRA records systems.

You're a Great Fit For This Role If You Have:
- High School diploma required. Bachelor’s degree in social work or related field, such as psychology, vocational rehabilitation, or occupational rehabilitation, preferred.
- One (1) year of human services or related experience.
- Bilingual (English Spanish) preferred.
Compensation
$21.16-$22.12/hr./$44k-$46k annually
