PRIMARY LOCATION(s): Will have a cubicle workstation in the main office but will travel to all sites as needed

PRIMARY SCHEDULE: Generally, Monday through Friday 9am-5pm; Some evenings and weekends; Flexibility is required as schedule is dependent on programmatic needs (ie: training, med pass observations, covering for call offs, emergencies, ie: individual in hospital, rehab.etc)

REPORTS TO: Nurse / Health Services Manager

QUALIFICATIONS: 2 years full-time DSP experience, NADSP credentialed a plus!

Willing to work with all residents at all sites as needed. Flexibility. Strong computer skills. Robust organizational and time management skills. Ability to communicate effectively and compassionately with individuals, families, and interdisciplinary teams (internal and external)

Proficient knowledge of the following

Office of Developmental Programs (ODP) regulations

Incident Management

Medication Administration/Documentation

Positive Behavior Support

Abuse prevention

HIPAA and confidentiality

CPR/First Aid

Therap - compliant notes, SComm, EMAR, etc.

JOB DESCRIPTION:

GENERAL:

Ability to work as a team member, follow directions, and agree to supervision by Nurse / Health Services Manager.

Willingness to carry out the mission and support the values of the Emmaus Community

Maintain and promote respectful and meaningful relationships with the residents.

Help to ensure compliance with all State and County mandated regulations (6100s 6400s and 6500s)

Provide leadership and model best direct care practices for new staff

MEDICATION AND HEALTH RELATED TRAINING:

Administer medication and participate in medication training of new staff

Will take Medication Administration Train the Trainer course within 6 months of acceptance of position

Works closely with RN (and other trainers/practicum observers) on Medication Training new staff - WILL BE PRIMARY MED TRAINER once acclimated to training structure.

Researches and recommends (may create or coordinate) training on specific healthcare needs (in-person, on-line, etc.)

Ensures all health-related Relias Trainings (CPR, 1st Aid, Med Admin) are uploaded in a timely manner.

Helps to plan and conduct health related activities for Emmaus Enrichment

Assists in educating residents about the importance of medical appointments and ensures documentation regarding all refusals

Health Risk Screening Tool BACK UP rater

Assists with compiling reports and data from HRST

Will take HRST rater training (within 6 months of acceptance of position)

HEALTHCARE, TRACKING, AND DOCUMENTATION:

Assists with eMAR configuration and oversight

Adding new medications, updating dosages, discontinuing medications

Works alongside RA/SDSP to review eMARs on a routine basis to ensure complete and accurate documentation of medication administration

Acts as a liaison between PDC and Emmaus when RN is not available (resolving prescription issues/delivery issues, etc)

Assists Residential Advisors/SDSPs in resolving and managing medication issues

Assist in semi- annual physician order review and distribution

Ensures accurate utilization of appointment tracking/health tracking features in Therap and other healthcare data entry

Attends medical appointments with residents as needed

Ensures all follow up is completed (lab orders, paperwork filed appropriately, changes communicated to team, future appointments scheduled, etc.)

Emergency Medical Care – works with Point Person/Nurse to ensure all relevant documentation is collected, filed, and shared with appropriate staff; ensures all follow up is completed.

Visits individuals in hospital / rehab facilities / etc. to ensure proper care planning; assist with discharge planning and follow up

Coordinate / schedule supplemental habilitation as needed when an individual is inpatient

Works with team to plan quality post-acute care (including relevant training)

Track residents’ annual physical due dates and TDAP (tetanus) vaccination to ensure all are completed within the required timeframes.

Provide RAs/SDSPs with reminder emails, collect completed physicals, review for completion/accuracy, then forward to the RN for review.

Periodically assists the agency nurse in reviewing all medical books to ensure follow up and documentation is completed and filed, and that records are up to date.

OTHER:

Participates in the On Call Rotation (after 3 months of accepting the position)

Attend staff meetings/trainings as required

Assists with other healthcare related activities that may need attention (ie: assist RN in conducting health related assessments such as fall prevention, safe eating, positioning, etc.; policy review and development)

Works with RN regarding Performance Based Contracting activities

Assists with incident management/entering incidents into EIM (primarily medication errors). Training will be provided

Provides feedback to staff responsible, resolves issues related to medication errors
