Position Summary:
The Case Manager/ Outreach Coordinator is responsible for assisting families of current and potential students to assess, access and manage community resources to meet their educational and financial needs. Serves as a family advocate, and liaison between family/client and community services and a support system for families within the child development center, and the community. Incumbent in this position demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.
Duties and Responsibilities
The Case Manager is responsible for:
- Responsible for enrollment and coordination of services for children in the program.
- Ensure referrals are addressed within the contract requirements.
- Provide primary service coordination of children enrolled in program.
- Coordinate and schedule appointments for intake with parents.
- Conduct psychosocial assessments of new children and families entering the program.
- Conduct autism screenings, diagnostic evaluations, and target parental counseling interventions to families in the communities.
- Provide information, referral and follow up services to all the children and families being served at the Center.
- Identify client data base to support program implementation and enhance data tracking, reporting and productivity.
- Work in conjunction with the Center Director to support program as needed.
- Streamline participant application process and maintain up to date waiting lists.
- Participate in outreach and community events to promote the program
- Receive referrals from community agencies and internal team members; understand the verification process through websites.
- Assist families in navigating and accessing community resources available to them and coordinate services.
- Serve as liaison between patients and families, affiliates, and community agencies; initiate contact with patient/family/representative to assess necessary assistance and prioritization of needs.
- Work in conjunction with other program staff to ensure educational, financial, and transitional needs are met.
- Support program administration with the documentation required to obtain and or maintain licenses, accreditations and permits to secure services are not interrupted.
- Ensure completion of all paperwork relating to the childcare food program.
- Maintain positive relationships with funding sources/groups, regulatory and community agencies.
- Attend in-service training, supervision and meetings as required.
- Performs all other duties as assigned by supervisor.
Physical Demands:
- Regular to frequent requirement, and as needed, to lift boxes (up to 50 lbs.), including bending, stooping, stretching, walking, squatting, pushing, pulling, kneeling or sitting on the floor.
- To conduct activities and interact with clients in a meaningful way, some physical exertion such as the following physical demands are required:
- The noise level can be moderate to loud.
- Detecting unsafe situations by sight or sound.
- Moving quickly to intervene in unsafe situations.
Educational & Experiential Requirements:
- Minimum Associate’s degree in Social Work, Education, or Management. Preferred Bachelor’s degree in identified areas.
- Required: Bi-lingual English and Spanish.
- Required: Minimum of one (1) year or experience working with vulnerable population.
- Preferred: 45 DCF hours
- Preferred: Knowledge of government and community-based fiscal programs and health insurance for families in need of coverage.
- Proficiency with Microsoft Office applications and data management software required.
- Ability to effectively communicate, verbally and written, with diverse (educational background, cultural, race and ethnic) groups or individuals.
- Customer service-oriented, strong organizational, time management, attention-to-detail and follow-up skills.
- High level of accuracy and analytical skills.
- Working knowledge of services to infants / young children with special needs and their families.
- Ability to maintain confidentiality regarding personal information of participants and staff.
- Desire to improve the lives of people with disabilities and or special needs.
- Ability to work under pressure, maintain accuracy and meet deadlines.
- Willingness and ability to commute to different centers.
- Willingness and ability to work weekends
Other:
- Ensure the safety of all staff, volunteers, visitors and individuals receiving services at Easter Seals.
- Act in compliance with Medical and Non-Medical Emergency Procedures.
- Assist individuals in evacuating building during emergency situations to include but not limited to: fire drills and external disaster drills. Specific duties to be assigned by immediate supervisor.
- Assume assigned responsibility for non-clinical duties during disaster and/or drills including but not limited to safety of files/office areas, closure of doors.
- Successful completion of background process which includes: Local background check, Level 2 clearance (FBI, FDLE), clear TB test, medical clearance, drug test and reference checks.
- Work a flexible schedule as required.
- Adhere to the use of Universal Health & Safety Precautions as applicable
