Impact Health is seeking a compassionate and dedicated Palliative Care Nurse Practitioner to join our growing healthcare team in Philadelphia, PA!!

Compensation and Benefits

Earning potential: $115,000–$200,000 (salary based on experience + easily achieved monthly bonuses)
Flexible Schedule: Monday – Friday. No on-call, weekends, or holidays.

Key Responsibilities

Deliver person-centered care using evidence-based practices
Collaborate with interdisciplinary facility teams
Conduct assessments, develop treatment plans, and manage follow-up care
Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
Educate patients, families, and staff
Complete timely documentation (within 24 hours)
Compliance with facility, state and federal regulations/protocols.

QualificationsQualifications

Active license as a Nurse Practitioner (NP)
Minimum 2 years of post-certification experience
Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
Proficient with EHRs and clinical technology
Strong communicator with a collaborative mindset
