Licensed Practical Nurse (LPN) – Medical Respite Program

We are seeking a compassionate, organized, and clinically strong Licensed Practical Nurse (LPN) to join our Medical Respite Program on a part-time basis. This role supports residents in a short-term residential setting by providing direct nursing care, administering medications and treatments, monitoring health status, and helping promote a safe, respectful, and person-centered care environment. This is an excellent opportunity for an LPN who is committed to serving medically vulnerable individuals and working as part of an interdisciplinary team.

Schedule

Tuesday and Thursday 4:30pm-8:30pm; Saturday and Sunday 10am-4pm (20 Hours)

Key Responsibilities

Administer prescribed medications and treatments in accordance with medical orders, policy, and scope of practice
Monitor residents’ health status and promptly report changes in condition
Document care accurately, including medication administration, treatments, and nursing notes
Support admissions, transfers, and discharges, including health-related documentation and resident education
Provide routine nursing support, including vital signs, symptom monitoring, and basic treatment support
Follow infection prevention, safety, and resident care protocols
Collaborate with residents, families, providers, pharmacies, hospitals, and interdisciplinary staff

Qualifications
Qualifications

Current New York State Licensed Practical Nurse license required
Graduate of a state-approved practical nursing program
Experience in medical respite, residential care, shelter-based care, hospital, rehabilitation, long-term care, home care, or a similar clinical setting preferred
Experience with medication administration, wound and skin care, chronic disease support, and clinical documentation preferred
Strong communication, documentation, and teamwork skills

Commitment to resident dignity, confidentiality, and person-centered care

Our Medical Respite program provides short-term supportive housing and case management services to multiply-challenged patients leaving hospital care without a viable home in which to recuperate.

Our Life is Precious™ Program, which opened in 2008, provides non-clinical services to girls’ ages 12 to 17 in the Bronx, Brooklyn, Queens, Manhattan, Poughkeepsie, Yonkers, Hempstead, and Amsterdam. Activities include academic support, creative arts therapies, music lessons, wellness activities and concrete family services, are designed to reduce the risk factors associated with Latina adolescent suicide.
