The Program Health Aide position provides assistance in daily living and personal care services to meet patient’s needs. Under the supervision of the Senior Director of Programs, the Program Health Aide will provide routine personalized care. Essential Duties

Observe, monitor and record patient condition. Including physical and mental condition, exercise, as well as intake and output

Notify care team and clinical supervisor of changing or unusual conditions

Assist in personal care by maintaining hygiene regimens such as bathing, toileting, dressing and grooming

Support patients by providing housekeeping, laundry services, shopping for food, preparing and serving meals, running errands and other household requirements

Educate patients on how to care for themselves by teaching them how to properly use a cane/ walker, self-administration of medication, special utensils to eat and equipment for personal hygiene, as necessary

Maintain and keep patients living area clean and orderly

Advise on nutrition. Follow prescribed dietary requirements and nutrition standards

Follow plan of care and assist in exercise

Assist in the escorting of patients outside of the facility

Perform range of motion and other simple extensional therapy services

Identify new recreational opportunities or activities to help patients remain active

Maintain strong working relationships with patient to maintain trust

Maintain a safe, secure and healthy patient environment by following security precautions

Adhere to Comunilife professional standards, home care policies and procedures

QualificationsEducation and Experience Requirement(s):

High School Diploma or GED

CPR, First Aid and Crisis Prevention certification

At least one-year experience providing home health aide services. Experience supporting individuals with disabilities, mental illnesses or challenging behaviors

At least one-year experience with utilizing electronic medical record systems

One year of relevant community-based residential management or social service agency experience

Bilingual in Spanish preferred

Our Medical Respite program provides short-term supportive housing and case management services to multiply-challenged patients leaving hospital care without a viable home in which to recuperate.

Comunilife is a leader in the field of Latina adolescent suicide prevention. Our Life is Precious™ Program, which opened in 2008, provides non-clinical services to girls’ ages 12 to 17 in the Bronx, Brooklyn, Queens, Manhattan, Poughkeepsie, Yonkers, Hempstead, and Amsterdam. Activities include academic support, creative arts therapies, music lessons, wellness activities and concrete family services, are designed to reduce the risk factors associated with Latina adolescent suicide.
