The Assistant Program Director (APD) of Health Home Care Coordination provides operational, and clinical leadership for the agency’s Health Home Care Coordination Program. This role ensures that high-need Medicaid members receive integrated, person-centered, and outcome-driven care coordination services in alignment with New York State Department of Health (NYS DOH), Health Home Lead agencies, and Managed Care Organization (MCO) requirements.
The APD oversees the full continuum of program operations, including staff supervision, quality assurance, compliance, and performance management — with a particular focus on meeting billable service targets, documentation standards, and program sustainability. This position is eligible for Comunilife's generous benefit and paid time off package.
Essential Duties
Program Oversight & Leadership
Provide day-to-day oversight of all Health Home Care Coordination activities, ensuring consistent delivery of high-quality, person-centered services.
Monitor caseload assignments, service utilization, and billing activity to ensure that monthly billable targets and productivity goals are achieved.
Develop and implement systems to track staff performance, client engagement, and compliance with Health Home program metrics (e.g., outreach, enrollment, engagement, and active care management).
Collaborate with Finance and Compliance departments to monitor program budgets, billing integrity, and revenue projections.
Compliance & Quality Assurance
Ensure all client documentation — including Assessments, Plans of Care, Progress Notes, and Consent Forms — meets timeliness, accuracy, and quality standards required by NYS DOH, MCOs, and Health Home Leads.
Conduct regular chart audits, quality reviews, and utilization monitoring to identify trends and implement corrective actions as needed.
Lead responses to Health Home Lead audits, MCO reviews, and state monitoring visits.
Develop and update internal policies and workflows to align with evolving Medicaid and Health Home regulations.

Staff Supervision & Development
Ensure care coordinators complete 5 services per day to ensure completion of their caseload. Provide leadership and clinical guidance to Care Coordinators.
Conduct regular supervision meetings to review caseloads, service goals, and client progress.
Support ongoing staff training in trauma-informed care, motivational interviewing, harm reduction, health literacy, and documentation compliance.
Foster a supportive team culture that values accountability, professional growth, and excellence in client care.
Qualifications
Education and Experience Requirement(s)
Master’s degree in Social Work, Nursing, Psychology, Public Health, or a related human services field required. Experience
Minimum 5–7 years of progressive management experience in Health Home, Care Coordination, or Medicaid-funded care management programs.
Demonstrated success in meeting billable service goals, managing compliance, and leading multidisciplinary teams within a nonprofit or healthcare setting.
Strong understanding of NYS DOH Health Home standards, MAPP/HHTS workflows, and managed care billing requirements.
Experience using Foothold RMA or comparable care management software.

Our Medical Respite program provides short-term supportive housing and case management services to multiply-challenged patients leaving hospital care without a viable home in which to recuperate.
Comunilife is a leader in the field of Latina adolescent suicide prevention. Our Life is Precious™ Program, which opened in 2008, provides non-clinical services to girls’ ages 12 to 17 in the Bronx, Brooklyn, Queens, Manhattan, Poughkeepsie, Yonkers, Hempstead, and Amsterdam. Activities include academic support, creative arts therapies, music lessons, wellness activities and concrete family services, are designed to reduce the risk factors associated with Latina adolescent suicide.
