The Herma Heart Institute has an opportunity for a nurse to join the Adult Congenital Heart Disease (ACHD) team! This nurse will be paired with a provider supporting both an ACHD and pediatric clinic seeing patients ranging from infancy to 18+ years. In addition, this nurse will support our transition program for our pediatric cardiology patients 16 years and over.

Hours: Four 10-hour days 7am-5pm (no weekends or holiday's)

What you will do:

The Adult Congenital Heart Disease Nurse will be a member of the Herma Heart Institute Adult Congenital Heart Disease Program. This individual will provide and help coordinate care to adult patients with suspected or diagnosed congenital heart disease, in a manner that supports the continuity of care for this diverse and complex population. The nurse will also assist in program development and research initiatives in effort to improve the care and outcomes of all adults with congenital heart disease across the state of Wisconsin and Midwest. In addition, this nurse will provide care to our pediatric patients through a bi-monthly pediatric focused clinic.

Job Summary

Herma Heart Center Adult Congenital Heart Disease Program. This individual will provide and help coordinate care to adult patients with suspected or diagnosed congenital heart disease, in a manner that supports the continuity of care for this diverse and complex population. The nurse will also assist in program development and research initiatives in effort to improve the care and outcomes of all adults with congenital heart disease across the state of Wisconsin and Midwest.

Essential Functions

- Program Development: In collaboration with the Medical Director, Manager, and other ACHD team members develops implements, manages, markets and evaluates a program plan.
- Nursing Process: utilizes the nursing process when providing care to patients or consulting with other nurses or physicians; assesses and responds to patients’ physiologic and psychosocial status, including the appropriateness for procedures or treatment modalities; makes recommendations for procedural changes; assesses and responds to patients needs before, during, and after procedures and treatments; makes decisions about procedural or equipment modifications based upon assessments; negotiates systems to implement change;
- Case Management: uses case management as a strategy to assist in managing a population; collaborates with a multidisciplinary team in the establishment of goals and implementation of the plan of care; identifies priorities and realistic outcomes for intervention; identifies and communicates potential solutions to variances; implements identified solutions, negotiates systems, plans and directs the implementation of change related to program plans and standards.
- Organizational Responsibilities: takes a role in designing and implementing activities that support the mission and strategic objectives of the Adult Congenital Heart Disease Program.
- Teaching: identifies educational needs of patients and families, staff and community, develops formal teaching plan content and presents educational programs and conferences; communicates content for informal teaching; acts as resource for patients/families, staff and community; incorporates concepts of families and social interactive theory to assess and resolve barriers to learning.
- Discharge Planning: assesses patient/family needs related to discharge as applicable; offers input and additional resources in planning for discharge needs for this patient population.
- Administrative Responsibilities: assists in setting expected standards of performance within specialty; assists with evaluation of staff; contributes to the evaluation of orientation and on-going staff development needs; participates in orientation of staff to this specialty; assists with analysis and implementation of systems to utilize available resources in an efficient, cost effective manner.
- Consultation/Collaboration: acts as consultant to staff and physicians; provides expert advice, which incorporates the broader aspects of patients/families response to illness/injury; provides leadership in the assessment, development and implementation of policies and procedures, which are directly and indirectly related to the area of specialization; seeks opportunities for providing information/initiating consultation; assists with the development of new and advanced skills that are required to support the procedures within this area of specialization; assists in coordination of delivery of care and/or discharge by enlisting other resources.
- Research: applies current research findings in own practice; introduces current research into clinical practice; develops research proposals and implements proposals as appropriate, analyzes data and communicates findings through presentations and publications; participates in developing grants.
- Quality Improvement: Utilizes the PDSA model in the ongoing Quality Improvement initiatives for patient population.

What you will need:

• Requires a Bachelor's degree in Nursing and three to four years nursing experience, in order to develop a comprehensive understanding of patient care within a defined patient population, and gain knowledge and skill in nursing procedures and standards.

• Current CPR certification ' Basic Life Support (BLS) for Healthcare Providers through American Heart or American Red Cross is required.

• ACLS (Advanced Cardiovascular Life Support) certification - American Heart Association (AHA) training will be required within the first year of joining our team.

• Cardiac experience highly preferred

• Adult experience highly preferred
