Salary: $52,000.00 / year (Salaried)
Schedule: Monday – Friday, 9:00 AM – 5:00 PM (Includes weekend on-call responsibilities)

We are seeking a dedicated and experienced Shared Living Homefinder to support our growing Shared Living program (currently serving 10 individuals). Based out of our West Springfield office with travel across Hampden County, you will determine provider eligibility, match individuals with intellectual disabilities and brain injuries with safe, stable host homes, and ensure ongoing regulatory compliance.
The Opportunity: Connect, Protect, and Empower:
As a Shared Living Homefinder, you serve as the bridge between individuals, Shared Living Providers, and multidisciplinary care teams. You will ensure host homes maintain a supportive, person-centered atmosphere while adhering to all state regulations and safety safeguards.
Key Responsibilities:
- Eligibility & Placement: Evaluate and determine initial eligibility for potential Shared Living Providers; place individuals with intellectual disabilities and brain injuries in safe, nurturing home environments.
- Monthly Visits & Compliance: Complete ongoing monthly home visits throughout Hampden County to ensure provider homes continuously meet Department of Developmental Services (DDS) regulations and environmental safeguards.
- Collaborative Care: Work closely with Shared Living Providers, individuals' medical care teams, and outside vendors to coordinate holistic support and services.
- Crisis Response: Respond effectively to crisis situations and participate in a weekend on-call rotation to support participants and providers as needed.
Requirements:
- Language Preference: Bilingual (English/Spanish) skills are ideally preferred.
- Experience: At least 5 years of experience serving individuals with intellectual disabilities is required.
- Education: Bachelor’s degree preferred (or equivalent human services experience).
- Certifications: Active Medication Administration Program (MAP) certification required.
- Logistics: Valid driver’s license and a reliable vehicle for local travel and site visits throughout Hampden County.
- Skills: Strong problem-solving abilities and excellent verbal and written communication skills.
