JOB SUMMARY: The Case Manager provides a range of comprehensive community-based case management services, including housing services and trauma-informed case management for individuals who are homeless and/or returning to the community from incarceration. The position serves as a liaison to the District of Columbia Department of Corrections, READY Center, Anchor Mental Health (Core Service Agency), and Housing, to identify and coordinate community resources and make the appropriate referrals. The Case Manager establishes and maintains effective working relationships among area residents, and property owners in the community. 

ESSENTIAL DUTIES and RESPONSIBILITIES:  

  • Interview and assess the needs for direct services of Returning Citizens (post-release) and individuals referred by jails or prisons including the DC Jail before discharge (pre-release), primarily in the District of Columbia, who have mental health needs and/or a history of addictive behaviors.  
  • Meet with community partners, such as the Resources to Empower and Develop You (READY) Center, District of Columbia Department of Corrections (DCDOC), Court Services and Offender Supervision Agency for the District of Columbia (CSOSA), Office of Returning Citizens Affairs (ORCA) and other partners to identify areas for collaboration and develop plans for coordinating services. 
  • Ensure participants engage in group sessions that allow them to address and improve personal aspects of their lives such as financial literacy, employment readiness, identity development, relationships, family dynamics, and conflict resolution. 
  • Maintain complete and accurate case records to include entering data in the database within 48 hours of contact/service 
  • Ensure appropriate match between volunteer mentors and interested clients through well-defined recruitment, selection, and retention strategy.
  • Foster collaborative relationships with organizations and service providers aimed at sharing resources that support men transitioning to the community. 
  • Collect and track data elements from each participant. 
  • Perform other job-related duties as assigned.

Requirements

EDUCATION and EXPERIENCE:

  • Bachelor’s degree in social work, human services, or related field.
  • For social work degree - social work licensure in the appropriate jurisdiction is required. 
  • 3 years of experience working with incarcerated adults including those with mental disorders 
  • Must be able to pass a background check to gain access to secured facilities.

SKILLS and COMPETENCIES:

  • Driving is required. Must have a valid driver’s license and clean driving record. Must have own transportation. 
  • At least 50% of the time is spent in the community/virtually performing information sessions, making contact with state and local jails and correctional facilities, recruiting mentors, and identifying resources for the client.
