Job Summary: 

The Family Support program strives to achieve the goal of helping families stay together through developing families’ natural capacity to meet the needs of family members; offering additional supports such as staff resources, goods and services, and financial assistance; and enhancing the capacity of communities to value and support people with disabilities and their families. Guided by these principles and through case management and coordination, the Family Support Navigator ensures that all families are treated with dignity and respect and given opportunity to reach the goal of services provided. 

This is a non-exempt position.

Supervisory Responsibilities: Position does not have supervision responsibility of staff

Essential Functions of Position: 

Case Management / Coordination 

  • Responds to inquiries to the Family Support Center as a first responder 
  • Provides case management to families through the Family Support Center including assessments, short term and extended navigation services, financial allocation support and resource assistance
  • Assists families in the development and implementation of Family Support Plans 
  • Provides training to family members utilizing intervention strategies in home/community
  • Facilitates support groups for families through the Family Support Center 
  • Recruits and trains staff for home based services 
  • Oversees home based services through supervision and case management of DDS cases 
  • Serves as liaison between family, in home worker and DDS
  • Provides assistance in locating community resources and referrals as appropriate
  • Act as liaison with other service providers such as special education programs, health care providers
  • Maintains documentation according to program and funding requirements
  • Attends internal and external meetings as needed
  • Other duties as assigned by supervisor 

Qualifications:

  • BA/BS in social work or related field 
  • Five years experience in Human Services or related field preferred
  • Valid driver's license required
  • Use of personal vehicle needed for transportation to clients’ homes, appointments, meetings and to perform varied work duties as needed
  • Use of personal cellular telephone for work communication 
  • Bilingual in English and Spanish, Cape Verde and Portuguese preferred.

Homelessness/addiction programs: an overnight shelter, seasonal warming center, street outreach team, and supported housing with in-reach services for chronically homeless adults.

Behavioral Health Integration services: a state-funded Community Behavioral Health Clinic and two federally funded Certified Community Behavioral Health Clinics (in Boston and Cape Cod), integrating substance use and mental health services.

Community Partner and Health Home services for people with serious mental illness or long-term support needs, aiming to improve health outcomes through integrated healthcare and behavioral healthcare services.

Early Intervention Program promoting optimal development for children under three who are developmentally delayed, have a known disabling condition, or are at risk due to biological or environmental factors.
