Opening in Fall 2026, Avivo Village Central Minnesota is bringing Avivo's innovative approach to ending homelessness to St. Cloud.
As a Case Manager, Housing (Site Based), you'll help launch a brand-new program while supporting residents as they work toward housing stability, improved wellness, and long-term success. Through relationship-based case management, you'll help residents navigate barriers, identify goals, connect with resources, and take meaningful steps toward permanent housing.
This is an opportunity to help shape the resident experience from day one while working alongside a dedicated team committed to creating a safe, welcoming, and supportive community.
Founding Team Opportunity: Help establish the culture, services, and participant experience of Avivo Village Central Minnesota before opening in Fall 2026.

What You'll Do
Build trusting, person-centered relationships with residents and provide ongoing case management services
Complete assessments and help residents develop individualized goals focused on housing stability and overall well-being
Support residents in accessing healthcare, mental health services, benefits, employment resources, and community supports
Assist residents in identifying and overcoming barriers to maintaining long-term housing
Coordinate services with internal teams, landlords, service providers, and community partners
Support residents through transitions, crisis situations, and life changes while promoting stability and independence
Maintain timely and accurate documentation, assessments, and service plans
Monitor participant progress and adjust support strategies to meet changing needs
Collaborate with Resident Advocates, Team Leads, and program leadership to support positive resident outcomes
Promote Housing First, Wellness & Safety, Trauma-Informed Care, and person-centered practices throughout all services
QualificationsWhat You Bring
High school diploma or GED and relevant experience working in housing, homelessness services, behavioral health, or a related field
Associate degree or higher in human services, social work, psychology, behavioral health, or a related field preferred
Experience supporting individuals experiencing homelessness, mental health challenges, substance use disorders, or other complex barriers
Strong case management, relationship-building, and problem-solving skills
Ability to effectively engage individuals from diverse backgrounds and life experiences
Experience coordinating services and navigating community resources
Strong organizational skills and ability to manage multiple priorities
Experience with electronic health records and documentation systems preferred
Commitment to Housing First, Wellness & Safety, and person-centered services
Valid driver's license, proof of insurance, and reliable transportation
