Description
The Foster Care Case Manager ensures overall coordination and management of cases through assessment, service planning, linkage with services, and establishment of collaborative relationships. The case manager provides a majority of services in the community and in families’ homes. The foster care case manager is the primary liaison between the participants (child, parents, foster parents) and the various systems they interact with during the course of their involvement with DCFS. The foster care case manager; provides basic counseling/stabilization as needed with supervision and consultation; communicates about the case regularly with the supervisor and external service providers; keeps the supervisor informed of any incidents/situations that may have an adverse impact on the participants or the program; documents all relevant information and activities on a timely basis.
Requirements
- Bachelor’s degree from an accredited college or university in social work, psychology, sociology, or a related field.
- Minimum of 2 years direct service experience with infants, children or adolescents.
- Bilingual Spanish/English required.
- Must have daily use of a car, current insurance, valid driver’s license, and a driving record acceptable to Agency insurance carrier and DCFS.
- Training for Child Welfare license is provided.
