How you'll make an impact in this role
- Deliver compassionate, high-quality direct patient care that adheres to established best practices, ensuring comfort and safety throughout the patient’s stay.
- Manage the comprehensive patient care journey by implementing care plans, executing physician orders promptly, and meticulously documenting progress to ensure continuity of care.
- Monitor and evaluate patient conditions, communicating critical status updates to the multidisciplinary team to ensure timely and informed clinical intervention.
- Coordinate services across clinical disciplines, acting as a primary liaison to guarantee seamless, well-integrated care for optimal outcomes.
- Facilitate safe, smooth transitions by assessing discharge needs and collaborating with the healthcare team to ensure a successful return home.
What minimum qualifications you'll need
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
